Our Clients receive core services, which include food provision, recreation activities, individual and group counseling, educational assistance, housing assistance and placement, healthcare assistance, referrals, and transportation. Individual case management is an essential and required component. Case management includes formal assessment of each Client's needs, a comprehensive service plan based on this assessment, an on-going evaluation of the Client's progress, and follow-up service provisions. Programs include psychiatrists, nurses, service coordinators, program directors, team leaders, and clinicians from a range of social work, psychology, and addiction backgrounds. Both group and individual work focuses on employment, housing, illness management, substance-related issues, social skills, and activities of daily living. Imagine social workers, therapists, counselors, teachers, and other helping professionals coming alongside an at-risk community of single parents, foster children, blended families... to offer a series of family oriented activities in a non-threatening setting.
